Running a Practice-Based Study? Start Tomorrow

There is a strange moment in practice-based research when everything is ready to go but nothing happens. The question is important. The intervention is low-risk. The data are available. The team is ready. Everyone is waiting on a determination.

Most people assume that wait is unavoidable. For many practice-based projects, it isn’t.

The Teacups Don’t Need a Harness Check

Every ride at Disney World has some version of a safety check before you board. Space Mountain gets a full harness, a height check, and a Cast Member confirming the restraint before the car moves. The teacups get a lap bar and a glance to make sure nobody’s standing up.

Nobody wants it the other way around. But imagine if every ride in the park required the Space Mountain treatment. Every teacup, every carousel horse, every kiddie coaster, the same harness, the same review, the same line. The park wouldn’t be safer. It would just be slower, and by mid-afternoon nobody would have ridden anything at all.

That’s close to what happens when every research project, regardless of its actual risk, has to move through the process built for the projects that carry the most risk.

Traditional IRB infrastructure was built to protect human subjects, and that protection matters. But not every project presents the same level of risk or requires the same pathway.

Ethical oversight should match the risk of the work.

A randomized drug trial and a nurse manager evaluating whether a new discharge huddle improves workflow are both capable of producing valuable knowledge. They are not the same kind of research problem. They shouldn’t require the same regulatory journey. One is Space Mountain. The other is the teacups.

The Wait Was Never About Your Project

Most clinicians already know which kind of project they’re running. The delay rarely comes from not understanding the study. It comes from not knowing a faster lane exists for it, and standing in the standby line by default because that’s the only line anyone pointed to.

Different Rides, Different Lines, Same Park

RapidIRB is built around that same idea. Quality improvement and practice-based projects move through a QIPR determination. Low-risk research that meets federal exemption criteria moves through an exempt determination. Neither skips the safety check. Both are sized to the ride.

For a project in one of those two lanes, the wait isn’t weeks. It’s the time it takes to answer a few questions about the work. The study you assumed would take six weeks to even schedule is one you could start tomorrow.

A Practice Worth Trying

Before you assume your project is stuck in the standby line, ask which ride you’re actually on. If it’s the teacups, you may already be closer to starting than you think.

Reflection Question

What is the study you’ve been meaning to start, and what would it take to find out you’re already in the fast lane?
