Federal Audit Survival: How Proper IRB Documentation Saves Careers

By Alysia Adams, DNP, APRN, AGACNP-BC, CCRN, NE-BC

In many healthcare and research settings, IRB review is often treated as a step to complete before the real work begins.

But even after a study concludes, when data is collected, and results are implemented, there comes a moment that brings everything back into focus.
This critical juncture is the audit—the point when past decisions and documentation are revisited.
Audits are a routine part of research and compliance oversight. At the federal level, agencies such as the Office for Human Research Protections and the Food and Drug Administration conduct both for-cause and not-for-cause reviews. At the institutional level, audits may occur annually, periodically, or in response to specific triggers such as publications, grant activity, or internal quality reviews.
In addition to these external reviews, many organizations and platforms supporting research processes have adopted ongoing internal audit practices.
For example, RapidIRB conducts quarterly reviews and audits approximately 30% of studies to ensure continued alignment with federal criteria and documentation standards. Put simply, audits are not rare events; instead, they are an expected and recurring part of research and evaluation work.
When you receive notification—whether a request for documentation, a compliance review notice, or a follow-up tied to publication or report—what matters most is what happens next. At that point, the spotlight quickly shifts from the project’s outcomes to the underlying process.

When the Questions Start

Audit requests are rarely complicated but precise.
You may be asked to provide:
  • The IRB determination for the project
  • Documentation supporting the exempt or non-human subjects classification
  • Consent materials, if applicable
  • Protocols, tools, or data collection processes
  • Evidence of how decisions were made and by whom
There is often an assumption that if the work is low-risk, these questions will be easy to answer. Sometimes they are.
But when documentation is incomplete or when determinations were made informally, those questions become harder to answer with confidence.

Where Risk Quietly Enters

One of the most common patterns seen during audit review is not intentional noncompliance; it is informal decision-making. A project is thoughtfully developed. A team determines whether it is a quality improvement or an exempt activity. The work moves forward with good intent and meaningful outcomes. What is often missing is not judgment but documentation.

Without a structured and recorded determination:
  • There is no clear record of how the decision was made.
  • No alignment with federal criteria is demonstrated.
  • There is no external validation of the classification.
And in an audit, that absence matters. When using RapidIRB, you can use your dashboard to locate study documentation.

The Role of the Principal Investigator

A critical point that is sometimes overlooked is this: responsibility for compliance does not sit solely with an organization or an IRB.
It sits with the Principal Investigator (PI).
The PI is responsible for:
  • Ensuring the project is appropriately classified
  • Securing the correct level of review or determination
  • Maintaining documentation throughout the lifecycle of the work
  • Being able to produce that documentation when requested
Even with IRB or external services, the PI must demonstrate adherence to proper processes.
This is not about adding burden. It is about maintaining ownership of the work.

The Value of a Clear, Structured Process

In environments where IRB infrastructure is readily available, this process is often embedded into the workflow. In other settings, clinical operations, innovation teams, and independent projects, there is an opportunity to bring that same level of clarity and structure into the process.

Having a consistent approach to:
  • Determining project classification
  • Applying federal criteria
  • Generating formal documentation
creates a level of confidence that carries through the entire project lifecycle.

Solutions like RapidIRB are designed to support this process by offering structured, algorithm-driven determinations and immediate documentation that aligns with federal standards. This allows professionals to move forward with clarity, knowing that the foundation of their work is documented and defensible. 🔍 Federal Audit Readiness Checklist

Before moving forward with your work, ask yourself:

☐ I have clearly determined whether this is research, QI, or non-human subjects work
☐ I have documented that determination in a structured, formal way
☐ My determination aligns with federal definitions and criteria
☐ I have official documentation supporting exempt or non-human subjects status (if applicable)
☐ My consent process (if applicable) is clearly defined and documented
☐ All study materials and decisions are organized and accessible
☐ My documentation would be clear and defensible to an external reviewer
☐ I could produce all required documentation immediately if audited
